Cataracts are the leading cause of reversible blindness worldwide — yet most people don't realize they have one until it has meaningfully impacted their daily life. The encouraging truth: when treated at the right time, cataract surgery is one of the safest, most successful procedures in all of medicine.
What Exactly Is a Cataract?
Behind the iris — the colored part of your eye — sits a transparent, flexible structure called the crystalline lens. In a healthy eye, this lens is clear and focuses light precisely onto the retina, producing sharp vision. A cataract occurs when proteins within this lens begin to break down and clump together, causing the lens to cloud.
The word "cataract" comes from the Latin cataracta, meaning waterfall — a reference to how advanced cases can make vision appear as if you're looking through turbulent, milky water. The process is almost always gradual, unfolding over years rather than weeks.
There are three main types, categorized by where they develop on the lens:
Nuclear Cataract
Forms in the center (nucleus) of the lens. The most common age-related type. Causes gradual yellowing and progressive nearsightedness before blurring.
Cortical Cataract
Develops as spoke-like opacities in the outer lens cortex, working inward. Often causes significant glare and problems with contrast.
Posterior Subcapsular Cataract (PSC)
Forms at the back surface of the lens. Associated with steroid use and diabetes. Can affect reading vision and cause halos around lights early on.
Recognizing the Symptoms
Cataracts develop so slowly that many patients attribute early symptoms to normal aging or simply "needing new glasses." Knowing what to watch for helps you seek evaluation before vision loss becomes significant.
Blurry or Cloudy Vision
The most common early sign — objects appear hazy or out of focus, as if looking through a frosted window.
Increased Glare and Halos
Bright lights, headlights, and lamps may seem unusually dazzling or surrounded by halos, particularly at night.
Fading or Yellowing of Colors
Colors may appear less vivid or take on a yellowish or brownish tint as the lens discolors over time.
Frequent Prescription Changes
Needing new glasses or contacts more often than usual — or finding your current prescription suddenly feeling "off."
Double Vision in One Eye
Seeing a double or ghost image in a single eye, which may disappear as the cataract continues to develop.
Difficulty Seeing at Night
Reduced contrast sensitivity makes night driving and low-light environments increasingly difficult and uncomfortable.
Important: These symptoms can also indicate other eye conditions — including glaucoma and macular degeneration — that require different treatment. Never self-diagnose. A comprehensive eye exam is the only way to know for certain.
What Causes Cataracts?
While aging is the primary driver, several factors can accelerate lens clouding — or trigger cataracts in younger patients. Understanding your personal risk factors is valuable for both prevention and early detection.
Age
By far the most common cause. The eye's natural lens is made primarily of water and protein. Over decades, those proteins clump and cloud the lens.
UV Exposure
Cumulative, unprotected sun exposure accelerates lens protein breakdown. Wearing UV-blocking sunglasses matters more than most people realize.
Diabetes
High blood sugar levels alter the chemical composition of the lens, speeding cataract formation — often at a younger age.
Steroid Use
Long-term use of corticosteroid medications (oral, inhaled, or topical) is associated with posterior subcapsular cataracts.
Eye Injury or Surgery
Trauma to the eye or prior ocular surgery can trigger what's known as a traumatic or secondary cataract.
Smoking and Alcohol
Both have been independently linked to a higher risk and earlier onset of cataracts through oxidative stress.
How Are Cataracts Diagnosed?
Cataracts are detected during a comprehensive dilated eye examination. After dilating your pupils with eye drops, your physician uses a slit-lamp microscope to directly examine the lens for signs of clouding, assess the type of cataract, and evaluate its density.
Visual acuity testing, contrast sensitivity tests, and glare testing may also be performed to understand how much your functional vision is being affected. This information, alongside your lifestyle and visual demands, guides the conversation about timing for surgery.
When Should a Cataract Be Removed?
There is no medication, eye drop, or supplement that reverses or prevents cataract progression. Surgery is the only effective treatment — but it doesn't need to happen immediately upon diagnosis.
The right time for surgery is when your cataract meaningfully affects your quality of life: difficulty driving (especially at night), trouble reading, struggling with work or hobbies, or failing to meet vision requirements for a driver's license. We never push patients toward surgery before it's genuinely needed.
In the early stages, updated glasses or increased lighting may help manage symptoms. As cataracts mature, surgery becomes the only path to restored vision.
Modern Cataract Surgery: What to Expect
Today's cataract surgery is remarkably safe, fast, and effective. The standard procedure — phacoemulsification — takes approximately 10–20 minutes per eye and is performed under topical anesthesia (eye drops only; no needles, no stitches). Patients are awake but relaxed, and most experience little to no discomfort.
Here's what happens step by step:
A Micro-Incision Is Made
A tiny opening (roughly 2–3mm) is created at the edge of the cornea — so small it typically seals on its own without sutures.
The Clouded Lens Is Removed
An ultrasonic probe is inserted to break up (emulsify) the clouded lens into microscopic fragments, which are then gently suctioned out.
An Intraocular Lens (IOL) Is Implanted
A clear artificial lens — folded to fit through the micro-incision — is inserted into the same capsule that held your natural lens, where it unfolds into position.
Recovery Begins Immediately
Most patients notice significantly improved vision within 24–48 hours. Full healing typically takes 4–6 weeks, during which protective eye drops are used.

A Note on "Secondary Cataracts"
Some patients notice their vision becoming cloudy again months or years after cataract surgery. This is not a returning cataract — it's a condition called posterior capsule opacification (PCO), where the thin membrane behind the IOL becomes hazy.
PCO is easily treated in-office with a quick, painless laser procedure called a YAG laser capsulotomy. It takes only a few minutes, requires no incisions, and vision typically improves within hours.
